A 42-year-old woman was admitted for a complex partial seizure. At age 18 years, she had transient diplopia, which was considered a simple strabismus. Current examination revealed a left VI cranial nerve palsy and right arm hyperreflexia. Neuroimaging showed multiple intraparenchymal cystic lesions in white matter (figure), multiple calcifications in basal ganglia, and extensive bilateral leuko...

A 55-year-old man with carcinoma sigmoid colon (fluorouracil [5-FU]: cumulative dose of 6,600 mg/m; and oxaliplatinum: 245 mg/m) presented with encephalopathy and pancerebellar involvement 2 weeks after receiving a third cycle of chemotherapy. Brain MRI showed diffusion restriction in bilateral deep white matter, cerebellar peduncles, and splenium of corpus callosum (figures 1 and 2) suggestive...

A 67-year-old man presented with a several-year history of progressive gait ataxia, dysarthria, and a 2-Hz palatal tremor (video on theNeurology®Web site at Neurology. org). He did not have corticospinal tract signs. Cranial MRI revealed hypertrophic olivary degeneration on T2-weighted images (figure). Genetic testing revealed a transition C . T, nucleotide position 235, codon 79, resulting in ...

At age 8 months, an infant girl displayed rapid developmental regression. Family history, birth, and initial development were unremarkable. After hospital admission, cerebral MRI showed bilateral cystic lesions in the centrum semiovale. Follow-up imaging after 3 months demonstrated a dramatic progression in these alterations with demyelination of the supratentorial white matter (figure). Bioche...
